  • Nothing replaces your doctor’s expertise—always check with them before trying new things.

  • Health & Bloom Wellness Center is not liable for any health decisions I make based on group discussions.
  • Facilitators are not medical professionals and do not evaluate suggestions.
  • All shared information is personal opinion only, not professional advice.
  • I assume full responsibility for my own health choices and actions.
  • This group does not diagnose, treat, or cure any condition.
  • I understand that violating these guidelines may result in removal from the group.

*Submitting this form is not required, but to participate in any zoom meetings it must be checked off, signed, and emailed to Contact@healthnbloom.org or you will not receive any zoom link invitations or participate in any therapy services that we offer. Filling this form out will also formally verify your agreement and intent to participate respectfully.
